I acknowledge if chosen as a recipient of a scholarship from Members First Credit Union, the attached image, my name, and the college I am attending or plan to attend, may be used by M1 in social media, online and print publications, on the credit union’s website, and other media outlets.

I understand and agree that if selected as a scholarship recipient, I will remain enrolled for the full semester, maintain a GPA of at least 2.5, and submit a photo to be used for Members First Credit Union publications. If selected as a recipient, I authorize release of my name and/or likeness to Members First Credit Union. I certify that the above information is true and complete. I realize that if any of the above stated information is found to be false, or if I fail to meet the requirements, I forfeit scholarship consideration. I certify that I am not an employee or family member of an employee or official family member of Members First Credit Union. I understand that my completed application will not be returned to me.
